What are Elder Service Plans (ESPs)?
Elder Service Plans (ESPs) are "total care" plans that give medical and social services to frail elders so that they can live in their communities instead of in nursing homes. Elder Service Plans are part of PACE (Programs of All-inclusive Care for the Elderly).
Am I eligible?
Elder Service Plans are for seniors who are certified as eligible for nursing home care, but who want to live at home. You must be 55 or older and able to live safely at home. You must agree to get all of your health services from your ESP. Most services are provided in day health centers.
What benefits will I get?
You will get comprehensive medical and social services. Benefits include medical care, emergency care, prescription drugs, medical supplies and equipment, hospitalization, therapies, nutritional counseling, adult day care, transportation for covered services, in-home services, and family support. Services are usually provided in an adult day health center, but may be given in the elder's home or other facility. Services available 24 hours a day, 7 days a week, every day of the year.
The ESP program is covered by Medicare and MassHealth. If you have MassHealth, there are no fees for the program. Otherwise, you may have to pay a monthly premium.
How do I apply?
The only Elder Service Plan that serves the Worcester area is Summit ElderCare, sponsored by the Fallon Community Health Plan. Visit the Summit ElderCare web site, or call Summit ElderCare at 1-800-698-7566 for more information. You do not have to be a member of Fallon to enroll in Summit ElderCare.
