Home Care Programs

Home and Community-Based Waiver
The Home and Community-Based Services Waiver (HCBS) is a program for low-income Massachusetts residents who qualify for institutional care but want to live at home. The Waiver allows MassHealth-eligible frail seniors, people with intellectual disabilities, young children with autism, and adults with traumatic brain injuries to get needed health care and support services at home rather than in an institution.
In Worcester, Elder Services of Worcester (508-756-1545) handles HCBS Waivers for frail seniors. The Worcester Department of Developmental Services (508-792-7545) takes applications for Autism Waivers and Intellectual Disability Waivers. The Massachusetts Rehabilitation Commission (1-800-223-2559) handles Traumatic Brain Injury Waivers.

Kaileigh Mulligan Home Care Program
The Kaileigh Mulligan program is a MassHealth home care program for severely disabled children who require skilled nursing care or are dependent on assistive technology. Parent income or assets are not counted in determining financial eligibility for this program. This program allows children to receive care at home that is equal to the level of care they would receive in a nursing home or other medical facility.
To apply, call the Mass Health Enrollment Center at 1-800-408-1253 and ask for Kaileigh-Mulligan intake; or talk to your child's hospital social worker or case manager.

Massachusetts Home Care Program
The Massachusetts Home Care Program is a program of support services to help Massachusetts seniors live independently in their own homes. To qualify, seniors must meet income limits and show a need for support services. The Massachusetts Home Care Program is for seniors with low to moderate needs, who do not need daily nursing care. Frail seniors who are medically eligible for nursing facility care can get a higher level of service from the Enhanced Community Options Program (ECOP).

MassHealth Home Health Services
MassHealth pays home health agencies to provide home nursing care; home health aide care; and occupational, physical, and speech/language therapy at home to eligible MassHealth members. See MassHealth Home Health Services on MassResources.org for details.

MassHealth Personal Care Attendant (PCA) Program

Elder Service Plans / PACE
Elder Service Plans (ESPs) are "total care" plans that give medical and social services to frail elders so that they can live in their communities instead of in nursing homes. Services are provided at home, at adult day health centers, or other locations. ESP members must agree to get all covered services through their ESP plan. ESP services are free for seniors with MassHealth who are certified as eligible for nursing home care.

Senior Care Options
Senior Care Options (SCO) is a program that provides seniors with a complete package of health care and social services, including home health services. Senior Care Options (SCO) is for seniors 65 and older who are eligible for MassHealth Standard and agree to get all of their covered health services through the SCO plan. Seniors in all living situations, including seniors who are living independently, living at home with support services, or living in long-term care facilities, are eligible.

Home Care Assistance Program
The Home Care Assistance Program provides homemaking services for low-income disabled adults, age 18 to 59, who need help with tasks such as meal preparation, laundry, and house cleaning. The program helps people with disabilities live on their own and avoid going to a nursing home or other institution.

Supportive Housing
Supportive housing programs combine housing with support services. Supportive housing programs are for seniors and people with disabilities who need housing and cannot live independently. Housing options include private homes with host families, public and subsidized apartments with on-site services, congregate housing, and assisted living. All programs offer some support services such as meals, personal care, housekeeping, transportation, or activities.
